Power Of Attorney Form

Please fill out the Power of Attorney form.

  1. Choose an option to complete and sign
    Click on the gold button “Complete and Sign the Form Online”. A new page will open with the Power of Attorney ready for data entry.
  2. Enter the required information
    Fill in all mandatory fields with the requested details.
  3. Signing in the “Signee” section
    When you reach the “Power of Attorney Signee” section, click on the designated “Sign” field.
    In the pop-up window, select “Draw your signature.” On mobile, a “Draw” button may appear at the bottom instead—select that option.
    Draw your signature using a mouse (or your finger on mobile).
    Click “Sign” again to confirm.
  4. Completing the process
    After filling in all required fields, a green bar will appear at the bottom of the screen with the message:
    “You’ve completed all required fields. Review your document and click Finish.”
    (On mobile, only a green “Finish Signing” button will appear at the bottom.)
  5. Review the document and click “Finish” (or “Finish Signing” on mobile).
    That’s it!
    Make sure to complete all steps, including clicking the “Finish” button in the green bar, as the document will not be successfully submitted otherwise.

Mediation Authorization Form

Please fill out the Mediation Authorization Form.

  1. Choose an option to complete and sign
    Click on the gold button “Complete and sign the Form Online.” A new page will open with the Power of Attorney ready for data entry.
  2. Enter the required information
    Fill in all mandatory fields with the requested details.
  3. Signing in the “Signee” section
    When you reach the “Power of Attorney Signee” section, click on the designated “Sign” field.
    In the pop-up window, select “Draw your signature.” On mobile, a “Draw” button may appear at the bottom instead—select that option.
    Draw your signature using a mouse (or your finger on mobile).
    Click “Sign” again to confirm.
  4. Completing the process
    After filling in all required fields, a green bar will appear at the bottom of the screen with the message:
    “You’ve completed all required fields. Review your document and click Finish.”
    (On mobile, only a green “Finish Signing” button will appear at the bottom.)
  5. Review the document and click “Finish” (or “Finish Signing” on mobile).
    That’s it!
    Make sure to complete all steps, including clicking the “Finish” button in the green bar, as the document will not be successfully submitted otherwise.
